For seven days, use roughly three nights in Tbilisi, two in Kazbegi and two in Kakheti. With nine or ten days, add nights to Tbilisi and the region you care about most rather than automatically adding another destination.
Choose the base that fits the trip
Start with enough Tbilisi time that arrival day is not a sightseeing day
Three nights gives you an arrival evening and two useful city days. I would choose a neighborhood according to walking, slope and dinner plans, then keep the final Tbilisi morning simple before the mountain transfer.
If you plan to rent a car, decide whether collecting it for the entire city stay is actually useful. Many travelers will find it simpler to stay car-free in central Tbilisi and begin the vehicle portion when the regional route starts.
Give Kazbegi two nights so weather does not control the whole visit
A one-night Kazbegi stop can work, but two nights creates a far better margin for cloud, rain and the simple fact that the Georgian Military Highway is part of the journey. Stepantsminda is the practical base because it has the broadest local stay selection.
I would choose an outlying mountain property only after checking how I will reach dinner, trailheads and the next departure. With a driver or car, a remote view can be easy; without one, central convenience may improve the entire stay.

Finish in Kakheti only after deciding how the wine day will move
Telavi and Sighnaghi create different Kakheti trips. Telavi is a stronger general base for a wider regional route, while Sighnaghi is compact and atmospheric. The best choice depends on the wineries, villages and restaurants you actually intend to combine.
For a tasting-focused day, arrange a driver, tour or another non-driving plan before making a countryside stay non-refundable. Accommodation and transport are one decision here: a remote estate can be wonderful, but only if dinner, pickup and the next morning are already easy.

How many days do I need for Tbilisi, Kazbegi and Kakheti?
Seven days is workable with a 3/2/2 split. Nine or ten days feels more relaxed and lets you add time to the region you enjoy most.
Should I visit Kazbegi or Kakheti first?
Either direction can work. I would decide from the transport plan, weather window and where you need to finish rather than assuming one order is always better.
